Permitting Procedures/Instructions

THE FOLLOWING ITEMS ARE REQUIRED TO OBTAIN A BUILDING PERMIT FOR DECKS, SHEDS, PATIOS, GARAGES, ADDITIONS AND SWIMMING POOLS:


1.) Plot and lot numbers obtained from the Assessors office or a current tax bill.

2.) Plot plan showing the existing house, and all other structures including the proposed work indicating setbacks from the street, sidelines, and rear property line. Setbacks are measured from the furthest protrusions of the building, (stairs,
 overhangs, etc). This plot plan should also show the existing septic system and leaching field, (if applicable).

3.) Written approval from Board of Health (homes on Town Sewer are exempt from
this requirement).

4.) Written approval from the Conservation Commission.

5.) One (1) set of complete construction plans including a cross section of all materials to be used in the construction project.

6.) REScheck Insulation report (additions only). This can be obtained from a contractor, lumber yard or insulation contractor.

7.) Written approval from the Highway Department. (Garages and Additions only).

8.) Copy of Home Improvement Registration.

9.) Copy of Construction Supervisors License.

10.) Certificate of Insurance made out to the Town of Mattapoisett for Workmen's
Compensation and Liability Insurance. (Homeowners obtaining their own Building Permits are exempt from this process).


FLOOD PLAIN DISTRICTS

11.) All of the above requirements must be met including the following:

A. An addendum to the building permit application must be filled out
 completely, signed, notarized and returned with the permit if the construction costs approach 50% of the market value of the structure.

B. If the cost of your project exceeds the 50% figure, the existing structure MUST be elevated to the appropriate flood plain height.

C. All construction plans must be certified and stamped by a Registered Engineer or Architect that these plans meet all the requirements for building in the Flood Plain Districts, including Chapter 31 of the Massachusetts State Building Code.


CHECK OFF LIST

1.) Plot Plan.
2.) Board of Health (if applicable, see number 3 above).
3.) Conservation Commission.
4.) Highway Department Approval.
5.) Construction Plans.
6.) REScheck Insulation report.
7.) Most recent tax bill (flood plain projects only).
8.) Check made payable to the Town of Mattapoisett for the appropriate fee,
(see the Building Department staff for appropriate amount).

NOTE: Plumbing, Gas and Electrical Permits are required to be obtained separately
from the Building Department.

NOTE: The addition or relocation of a bedroom requires that the entire dwelling be electrically hard wired for smoke and carbon monoxide detectors.


NOTE: BUILDING PERMIT APPLICATIONS WILL NOT BE ACCEPTED UNTIL THE ABOVE REQUIREMENTS ARE COMPLETE.


THE FOLLOWING ITEMS ARE REQUIRED TO OBTAIN A BUILDING PERMIT FOR NEW HOMES AND COMMERCIAL BUILDINGS:

1.) Plot and lot numbers obtained from the Assessors office or a current tax bill.

2.) Plot plan showing the outline of your lot, where the building is located on the lot, and all setbacks (street, rear, and two sides), measured from the furthest protrusion of the building, (stairs, overhangs, bulkhead, etc.).

3.) If Town water and/or sewer are available, an approval slip is required from the Water/Sewer Department, 19 County Road, Mattapoisett.

4.) If your lot is serviced by a well, see the Board of Health for the location of this well on your property. A building permit may NOT be issued until the well is in place and the water is tested and approved by a water testing laboratory.

5.) Written approval from the Board of Health (homes on Town Sewer are exempt).

6.) Written approval from the Conservation Commission.

7.) Three (3) complete sets of construction plans.

8.) REScheck Insulation report. This can be obtained from a general contractor, lumber yard or insulation contractor.

9.) Manufacturers specifications on all LVL beams, trusses, and engineered lumber.

10.) Engineers or Architects stamped plan for any and all steel beams and any live
loads for manufactured lumber.

11.) Certificate of insurance made out to the Town of Mattapoisett for Workers
Compensation and Liability Insurance. (Homeowners obtaining their own Building Permit are exempt from this process).

12.) Copy of Construction Supervisors license.

13.) New construction plans must show any vent free gas appliances AND the location of the furnace.

14.) Written approval from the Fire Department (Commercial Buildings Only).

15.) Written approval from the Highway Department


FLOOD PLAIN DISTRICTS

16.) A. First floor elevation including basement and all living spaces must  be at the appropriate elevation above mean sea level.

B. No utilities may be located below the base flood elevation.

C. All construction plans must be certified by a Registered Engineer or Architect.

The Building Department will have all three copies of construction plans marked with the location of smoke and carbon monoxide detectors. It is important to note that one set of stamped, approved building plans remain on the job site throughout the project.

3.) Board of Health (septic system and well, if applicable).

4.) Copy of Sewage Disposal Plan. (If applicable).

5.) Conservation Commission.

6.) Three (3) sets of construction plans.

THE FOLLOWING ITEMS ARE ADDITIONALLY REQUIRED TO OBTAIN A BUILDING PERMIT FOR MODULAR HOMES:
1.) Foundation plan showing crushed stone, P.V.C. perimeter drains, damp proofing and vapor barrier.
2.) Foundation plan showing proper bracing of the walls before the modular boxes are set.
3.) Building Permit application must be obtained by a Licensed Construction Supervisor.
4.) Letter of certification for the installation contractor by the Modular Company.
5.) Manufacturers Building Plans showing the following:
A.  Third party approval stamp with dates showing current code compliance.
B.) Massachusetts architect seal of registration.
C.) Board of Building Regulations and Standards stamp of approval on cover page.
6.) Manufacturers site installation or set manual. This manual must show exactly how the modular will attach to the foundation.
7.) Annual re-certification notice from the Board of Building Regulations and Standards
